Reason for Outage is an independent engineering publication about AI systems in real operations.
It examines architecture, failure modes, and the decisions that shape a system: what it must do, the constraints it works within, and the evidence that supports a choice.
The aim is practical: help people building and operating AI systems ask better questions, weigh tradeoffs, and recognize risks before they reach production.
The opening series follows the development of agentic RAG, from source documents and retrieval to supported answers, tool use, and bounded actions.
